Healthcare in El Salvador: What Expats Need to Know

Healthcare is one of the pleasant surprises for expats in El Salvador. Private medical care is significantly cheaper than in the US or Western Europe, and quality at the top private hospitals is genuinely good. Here's what you need to know before you arrive.

Private vs. Public Healthcare

The public healthcare system (ISSS and Ministry of Health hospitals) is available to Salvadoran citizens and registered residents, but quality is inconsistent and wait times are long. Most expats use the private system exclusively. The cost difference between public and private is less dramatic than in many countries — private care is affordable enough that most expats don't bother with the public system.

Top Private Hospitals

Hospital de Diagnóstico and Hospital Médica Santa Ana are among the most recommended for expats in San Salvador. Hospital Centro de Emergencias and Clínica Médica Escalón are also well-regarded. These facilities have English-speaking staff, modern equipment, and doctors who trained internationally. A specialist consultation typically costs $40–$80 USD.

Health Insurance

International health insurance is strongly recommended. Plans from providers like Cigna Global, Allianz Care, or Aetna International run $100–$300/month depending on age and coverage level. These plans cover you in El Salvador and when traveling internationally — important if you plan to return home periodically. Local Salvadoran health insurance is available but has more limited coverage.

Medications

Pharmacies are plentiful and medications are significantly cheaper than in the US. Many drugs that require a prescription in the US are available over the counter in El Salvador. Bring a supply of any specialty medications you rely on, as not everything is available locally — but common medications are easy to find.
